Our Chiropractic Travel Cards were designed by a chiropractor. They are ideal for documenting treatment when access to a laptop is not practical. They are printed on 67# card stock for added durability. Select from two different layouts. A free sample of our chiropractic travel cards are available by email.
About the CTC-6
The Chiropractic Travel Card is ideal for documenting patient diagnosis, objective, treatment plan and assessment. This card allows for recording patient contact information including insurance data. Two-sided card allows you to record 40 visits/treatments. Printed on 7" x 11" white card stock for added durability.

About the CTC-11
The Chiropractic Travel Card is ideal for documenting patient diagnosis, objective, treatment plan and assessment. This card contains improvements from our CTC-6 Travel Card allowing for recording patient contact information and insurance data. The new format reduces insurance rejection rates when properly completed. Two-sided card allows you to record 12 visits/treatments. Printed on 8.5" x 11" white card stock for added durability.
